2. Viewing
You or your representative can come to see us, look around the home, walk through the gardens and talk to the staff and the other
residents. Why don't you arrange to come in for an afternoon, have a meal with us and get to know us better?
3. Assessment
If you would like to join us, we would invite you to complete an assessment of your needs, so we can agree with you and your other
care professionals the type and level of care that is required.
We will contact you and make an appointment for a home assessment as soon as we have a vacancy.
4. Contract
If we are able to meet your care needs we will send you a contract for the delivery of service.
The contract will let your room assignment, its weekly rate and the date you will be admitted into the home.
If you are happy with the contract, you can sign it and return it to us.
5. Trial period
You can stay with us for up to six weeks before you fully commit to staying long term. At any time during the first six weeks you can
let us know whether you are happy with the service delivered so far. During this period we can analyse how care needs are being met and
whether we will be equipped to carry this out permanently.
6. Key worker
When you join us, you will be assigned a Key Worker, who is responsible for monitoring all aspects of your care and keeping a record
of any decisions made about your care.
Your Key Worker, is your main carer who will always be up to date with your needs and will help the other members of the Woodlands team to be aware of any special needs you may have.
7. Periodic reviews of care plan
Periodically your Key Worker will review various aspects of your care with you and write up a plan of care.
From time to time, your relatives or representative will be asked to give us their comments on your care, so together we can do the best job possible for you.
8. Annual review
The annual review of your care is similar to your monthly review, except Woodlands review contracts and fees at this time also.
If there were any changes Woodlands would discuss them with you and your relatives or representative and send you a letter with all the relevant information.
